Application Process

You may Download the Application and score sheet electronically, or request an application be mailed to you at the end of November, 2007 by telephoning the Biological and Health Science Division Office at (650) 949-7538.

Complete and return the application including all additional documentation along with the score sheet to the Biology and Health Science Division. Applications are received annually. The college must receive all applications no later than 4:00 PM, May 1st. Only the current year application is acceptable.
